You must provide a reasonable accommodation to employees who can't take the vaccine (including for COVID-19) because of a disability or a sincerely held religious belief, according to the Americans with Disabilities Act (ADA) and Title VII of the Civil Rights Act of 1964. Exemptions may be available, however, to those who can demonstrate a medical condition or sincerely held religious belief barring them from complying with any mandate. This form has been updated in light of OSHA’s COVID-19 Vaccination and Testing emergency temporary standard.
